Hydrotest failure?

I’ve usually quipped that I’m not aware of any tank failing a hydrotest, but this may be one such case. Cross posting it here from LinkedIn in case you didn’t see it there and because I don’t think I can embed videos in this forum. I should have brought this up as the safety moment during our meeting.

It seems to me a vertical weld failure, now the why is difficult to determine without more information.
